This report describes the principles and
pathways that will operate across Tameside and Glossop to support
people to remain out of hospital, both in a personal residence and
in a care home.
The overarching principle is that the registered practice remains
the prime provider of general medical services during core care
hours and PCAS out of hours and the ICFT Intermediate Tier Services
will continue to support people who require district nursing or
more specialist care.
Two key pathways will be followed to reflect the opportunities that
the 24/7 support available through care homes bring.
Care home residents will have proactive and reactive support from
Digital Health alongside that of the registered GP and ITS.
This report does not set out the specific details of any
significant changes to existing services or new services. These
will be the subject of additional detailed reports as
required.
